PROCEDURE PERFORMED: Aortogram, left leg angiogram.

PREOPERATIVE DIAGNOSIS: Limiting claudication left leg.

DESCRIPTION OF PROCEDURE: The patient was brought to the Angiogram Suite and prepped and
draped in the usual fashion. After infiltration of 10 mL of 1% Lidocaine, the right femoral
artery was accessed using a micropuncture needle. A micropuncture wire was introduced and
the needle was exchanged for an eventually #4 French BITE TIP sheath. A Universal flush
catheter was advanced over a Benson wire and a standard AP aortogram was performed.

The AP aortogram showed single patent bilateral renal arteries. The infrarenal abdominal
aorta was widely patent. Next, the catheter was positioned in the aortic bifurcation and
standard RAO and LAO views were performed. These showed patent bilateral common iliac,
external iliac and hypogastric arteries.

Next, the left common iliac artery was cannulated and the catheter was advanced to the left
external iliac artery. A left leg angiogram was performed. This showed a total occlusion
left common femoral artery. The left profunda and superficial femoral arteries were patent.
The left above and below-knee popliteal arteries were patent. The left anterior tibial,
tibioperoneal trunk, peroneal and posterior tibial arteries were patent.

Following this, all catheters and wires were removed and pressure was held to achieve
hemostasis.

I agree with your codes, except for 75774. The catheter was not moved from the external iliac artery to a branch of that vascular family. Also add modifier -LT to 75710 and 36246.
