What is MC?

IF you are referring to Medicare by any chance, Medicare does not cover ROUTINE ophthalmology visits. So next question: What do you mean by "routine"?

This is for 92002/92004/92012/92014 only. Choose Local coverage determinations in upper left. Type in any of the 4 eye codes, check off the 'J4 box' and hit the search icon.Upper right hand corner shows icon 'printable copy'. Click on this icon before printing.
Scroll down to icd9 codes that meet medical necessity. Answer VPCATS question to yourself first-are these the codes I am talking about....
