I am getting rejections for Aranesp injections for my pulmonology providers. We use J0881 for the medication with the NDC for each amount. These differ per mcg. We also use modifier 59 if there are multiple injections except with Medicare we use modifier 76. Sometimes these are paid by insurance sometimes they are rejected. Anyone else use this medication or have this issue? Thanks for any information.
