Wiki Arrows etc. where is it documented?

We have a document that says arrows & other symbols should not be used in medical record documentation (e.g. up/down arrow with htn, lipids, etc). However, we cannot locate it in the Coding Clinic which is the listed source. Any direction as to where this might be found?
