My Dr is performing Fusion with graft from T5 TO THE PELVIS,
I HAVE COME UP WITH 22610 (T5-T6), 22614 X11 (T6-L5) IS THIS CORRECT , OR DO I NEED TO SEPARATE THE LUMBAR WITH 22612 AND BILL IT LIKE THIS 22610( T5-T6) 22614 X 7 (T6-L1) and then 22612 x4 (L1-L5) Do i code for the sacrum ?

So, is the fusion from T5-L5 plus SI Joint fusion? No, 22610 and 22612 should not be coded together. You should pick one for your primary and the rest would be add on 22614. 22612 has the higher RVU. Yes, SI joint fusion should be coded separately.
