Arthroscopic assisted AC Joint Reconstruction, unlisted code?

I'm confused on this one, and haven't coded anything like this before. Would this be an unlisted code or a 23550 since this was done with arthroscopic assistance? Op note is attached. Thank you for any help!

This is generally coded 23552. The op note describes an open incision, and arthroscopic assistance is not separately considered or reimbursable, nor do we really care whether 20% of the thing was arthroscopic or 80%.
You don't get a -22 modifier for this. A standard 23552 includes the work of graft harvest, graft fixation, and reconstruction of both the AC and CC ligaments.
