What is the proper coding for the following inpatient non medicare procedure?

Arthrotomy of the right total knee replacement, resection of the large patellar bone spur, revision of the patellar component with cementing by a Triathlon 9 x 33 mm symmetric patellar component, exchange of Optetrak tivial polyethylene, CCK size 3, 11mm which was exchanged to a size 3, 15mm and synovectomy of the right knee.
