When billing for an asc, does the cpt modifier (ex. -51)come before the hcpcs modifier (ex. Rt)? For example should the code be listed as 67113, -51, rt or 67113, rt, -51?
 
The 51 modifier is not an approved ASC modifier - unless the carrier you are billing requires a 51. If it does place the 51 then the [rt or lt] :-)
