I cannot find specific language around an ASC billing for anesthesia (01992, not materials). I know there is a CCI edit for this, but I believe this is specifically addressing the surgeon billing for these 2 codes, not the facility. Can anyone point me in a direction regarding this practice?
 
There are no modifiers on the billing, and I have no documenation, so I can't tell if it's a CRNA or MD; but my understanding is that there is supposed to be a separate bill with a separate NPI?
