How do I bill for rev 370-general anesthesia with CPT 45385 on for the technical charges on a CMS 1500 for the Ambulatory Surgical Center to Medicare? CMS 1500 requires a CPT or HCPCs with the charge. Or is the payment for the for CPT 45385 include the payment for general anesthesia?
