Hello coders-

All charts have a facesheet on them . It usually begins with the facesheet, then the H& P, then Doctors orders, and so on and on, usually the chart is made up and the coder , biller etc gets the chart and codes and bills it. Now how many times does the dx and cpt change from the booking that those codes were already on the facesheet. Are we suppose to change the facesheet to the proper codes that we as coders change because of a dx, or cpt code is different from booking, or does it really matter at all ?, please let me know -thanks

95% of the time the coding changes from the way that it was booked. No, you dont need to change your facesheet. Coding is/should be strictly based on the operative note and has no reflection of how it was booked.
