Hi guys, does Medicare reimburse ASC for 2 level median branch block 64493, 64494, we seem to be having some problems.

Medicare does reimburse this. There is no separate payment for CPT code 64494 as that is status indicator N1 which is a packaged code, so the reimbursement for the additional levels is included in the case rate; as I understand it the charge (and payment) for this would be included in the base code. What problems are you having?
