I work at an ASC and I need a double check on a new procedure we are doing. Please verify the ICD-9 and CPT codes listed are correct.

Pre-op & Post-op Dx:
L4-L5 Spinal Stenosis (724.02)
L4-L5 Degenerative Spondyolisthesis (738.4)

Procedure:
L4-L5 Prone Direct Lateral Interbody Discectomy and Fusion (22612 & 22533)
L4-L5 Placement of Lateral Interbody Cage (22851)
L4-L5 Local Autograft and Allograft Placement (20930 & 20936)
L4-L5 Posterior Instrumentation (22840)
L4-L5 Neural Foraminal Decompression (?????)

If anyone needs to see any of the op note let me know. I was told this reimbursement is very high.
