I am wondering if anyone else is billing for the ASQ form (ages and stages questionaire). Our office is now going to use this form, however, our doctors do not like the standar ASQ form and want to adjust the form. I am not comfortable billing for this because they have "adjusted" the ASQ form. Anyone have any thoughts on the standard ASQ form?
