Question Assistant at surgery billing help

Our nurse practitioner was the assistant at surgery with our physician. I billed it to Medicare as 27130 with the AS modifier and it was denied as "The procedure code is inconsistent with the procedure code". I looked in the Medicare Physician Fee Schedule Database, and it shows a 2 so I know that code allows an assistant surgeon. Does anyone have any help they can give me to get this paid?
 
Did you mean to say that the denial was that the procedure code is inconsistent with the modifier?
Did you bill a completely separate claim in the name of the NP; not putting it on the same claim as the surgeon? Did you make sure there are no errors on the surgeon claim, like the AS was accidentally appended? Did the assistant at surgery claim have all the correct modifiers and diagnosis codes which match the surgeon's (esp. laterality)?
Did you cut the fee on the assistant claim from the amount that was billed for the surgeon?
Are NPs allowed to be assistant at surgery in AZ for a THA?
