Wiki assistant surgeon

In Medicare Fee Schedule database the indicators in the assistant surgeon column are 0,1 and 2. I want to be sure I understand what each means.
Is this correct?

0= asst surg never paid
1= sometimes allowed
2= usually allowed

Modifier 80

Assistant surgery

0 Assistant surgeon may be paid with documentation.
1 Assistant surgeon cannot be paid.
2 Assistant surgeon can be paid.

With the 0 and 2 indicator use the -80 Modifier
