When coding for Asthma follow up visits, do you all charge separately for the Asthma Control Test? If so, do you use 96160 with Asthma dx or G9432 or G9434.

I use whatever diagnosis the provider uses for the patient. The test code is 96160 provided that you have the documentation to support this code.
