Hello,
I have an op report in which a general surgeon atempted an AV fistula at the wrist which was unsuccessful, physician discontinued due to uncertain if vein stenosis, complicated branch or torsion of vein abandoned site and closed site. Then , went on to the antecubital site and was successful performing the fistula. Now, I am entertaining the idea that I can code the attempted AV fistula with a modifier 53 , diagnosis code of 996.74, V64.1 and the successful AV fistula with modifier 59 , to indicate a separate incision. Am I correct? Please help.
