I need some help with this. I don't know if I can charge anything for this or not. This was being done in our own Cath. Lab, not in a hospital.

Procedure Performed:
Attemped Left SFA Laser Aterectmy- Unsuccessful

Indications:
Lifestyle limiting left calf claudication.

Hemodyanmics:
BSA (m2): 2.25
Hemoglobin (gm/dl): 15.1

HR (bpm): Air Rest

Procedue Notes:
Unable to gain access to left popliteal. Access with arterial return obtained, but could nver advance wire more than a centimeter or so from the needle tip.
Isovue 370

Equipment Used:

(10cm) 6 Fr Terumo
Starter 3mm J Tip (150cm) Boston Sci.
AngioDynamics 5 Fr. Standard Micro-Introducer set

Medications Given During Procedure:

Versed 2 mg IV
Demerol 25 mg IV
Demerol 25 mg IV
Oxygen: 2 L/min via nasal cannula
Demerol 25 mg IV
Versed 1 mg IV
Demerol 25 mg wasted
Versed 1mg IV

Conclusion:
Unsuccessful PTA due to inabilit to gain access.

Any help would be greatly appreciated.
