My coworker and I are developing our evaluation & management annual audit plan, and we are looking to see what attributes you auditors out there use. We have basic ones like missed billable service, supports higher code, insufficiency (hx,exam,mdm), no documentation for service, etc.

I would appreciate any ideas or other attributes that you currently use for your E&M audit.
