I can't recommend a radiology book but "Medical Record Auditor" by Deborah Grider for E/M is super. The AMA has a copyright on it.

I got the same book -- I got it through an AMA mailing so I'm sure you could go to their website. You save money if you are a member. Amazon might be cheaper, not sure.

I did purchase this book from Amazon. It was cheaper going through this site and it arrived within a respectable time frame.
