Can someone tell me the difference between when to code 36821 and a transposition procedure 36818-36820?

How will be dictation be different?

AV fistulas created with vein transposition (36818-36820) require 2 separate incisions and may or may utilize tunneling.
Direct anastomosis 36821 sometimes called Cimino uses only 1 incision and doesn't require tunneling or transposition.
