If the physician does an open thrombectomy of an AV fistula, finds a stenosis at the anastamosis on the venous limb and does a balloon angioplasty, introducing the catheter for the angioplasty through the same incision that was used for the open thrombectomy, how is this coded? Do you use 35460 for open venoplasty or G0393? (codes are from memory not book). Would you code the angioplasty at all? (pt is Medicare).
