Patient had an ahmed valve implantation -cpt code 66180 - with scleral path graf for reinforcement, cpt code 67255, with avastin injection into the vitreous cavity- not sure on this code - this is also done at An ASC
Can I ALSO code for the injection ?
Someone told me to use C9257-.25 MG
Patient had 1.25 mg
so if this is true do i bill c9257 x5
and what about the injection code ???

You would bill x5 for the Avastin.
The injection code (67028), you could try with a 59 modifier IF it was done through a different sclerotomy site.
