Office anesthesia

Does anyone know where I can find what anesthesia procedures Medicare will pay for when provided in an office setting? Any link or help would greatly be appreciated!!!

Example: Anesthesia doctors doing Arteriovenous Fistulas in an office setting for End Stage Renal Disease patients in a cardiovascular office. We are being denied by Medicare for Place of Service. The Cardiovascular doctor is being paid.
