I've never billed an injection that doesn't need the NDC code for the drug.

We only have to report NDC for certain payers. The majority of our (oncology) medications are stocked in multiple vial sizes and so have multiple NDCs. Linking the correct NDC to each drug in our billing system is a manual process and would require another FTE just for that if we were required to report to all payers. So we report only for the 2-3 plans that currently require it.

I learned that you can only bill for 1 unit of J3420. If you bill 2 units, both will deny.
