looking at the codes for balanitis and the ICD 10 book says to "code also" not "use additional code". What is the difference?
 
According to the ICD-10 guidelines the 'code also' instructs that 2 codes may be required to fully describe a condition. But it does not provide the sequencing direction so that will be for the coder to decide.
