I have a Medicare pt who had a band placed in 2008 and she would like to have the band removed (not successful) and have a gastric bypass. I checked the Medicare web site and I called Medicare to see if there is any specific criteria for a conversion and there is nothing in print. So, I'm wondering if anyone out there has billed Medicare for a conversion, did you follow the same criteria?, Have the pt sign an ABN? Any help is appreciated. Thanks
