If a patient has had a gastric bypass in the past and is now pregnant, would you code that as V45.86 (bariatric surgery status) or 649.2x? The physician has not entered a note stating this is NOT complicating her pregnancy.

Then I would recommend using the status code V45.86

You must use the pregnancy code as the provider has not stated it is not a complication nor will he as this is definitely a complicating factor. So 649.23 is the correct choice

I apologize, I misread the documentation. You do need to use the pregnancy code unless specifically told it isn't a complication.
