Hi,

One of our patients needs to see a podiatrist for basic nail care. He does not have any lower extremity peripheral vascular disease or diabetes. Last time, he had a home visit from the podiatry team, and insurance covered it. He's now considering going out to see a podiatrist.

I called Harvard, which follows Medicare guidelines, and it appears that he won’t be covered if it’s basic nail care and not medically necessary. I just want to confirm if this information is correct.

Why not dx L60 or L84 be used and if infection B35.1? Also debridement /cutting of nail CPT used ..see11719-11721 then add 99211 for nurse? If pt toe nails get too long this can cause the elder person problems but must be documented or his chronic condition N18 or E11 or skin condition. .Also a MD or NP or PA provider must order this done by nurse. ..then should be covered since attend doc/provider put on claim
