BC/BS bundled excision & repair, and skin tag removal

I billed as above for a skin tag removal on the neck and a separate lesion on the neck so two things were removed. The insurance only paid for 11200 (skin tag removal) and not the excision and repair.

I sent them medical records and they still uphelding the denial as bundled.

Did I do something wrong with the coding or are they just denying it just to deny it?

You've got the 59 modifier on the wrong code. It needs to be on the 11421.

Really? I saw that on the NCCI edits that 11421 was in column one and 11200 is on column two and I always thought that modifier 59 goes on column 2 codes.

Your eyes must have been deceiving you: 11200 is the column 1 code!

I mailed in with a corrected claim with modifier 59 on the 11421 and 11200. I should've placed it only on 11421 bu hope it gets paid anyway.
