BCBS DENIAL CODE 45

My 96372 and J1100 are both being denied by BCBS with rejection code 45.(My OC has a 25 modifer on it) Are there any
suggestions are to how to get this paid? Thank-you

CO 45 is not a denial, it is the indication of what the contractual adjustment should be. Can you send a snapshot of the EOB with no PHI? You can also email it to me at vanessa.moldovan@aapcnab.com. http://www.x12.org/codes/claim-adjustment-reason-codes/
