BCBS DENYING 64450 WHEN BILLED WITH 97110

I have a provider who performs peripheral nerve injections on a patient then the pt receives physical therapy by a PT on the same dos. Both services are documented separately and BCBS is denying the peripheral nerve injections as bundled to the physical therapy, stating the use of the modifier is not supported.

We bill as

Peripheral Nerve Block Injection to B Sural / Peroneal / Saphenous Nerves.
The patient was placed in the semi-recumbent position. The skin over-lying the aforementioned nerves were prepped with hydrogen peroxide using aseptic technique. A 27g 1 1/2 in needle introduced through the skin to the proximity of the targeted nerves without complications. Following a negative aspiration, 1 ml of 0.25% Bupivicane was injected into each site without resistance. The needle was removed with the tip intact. Patient tolerated the procedure well with out complication or excessive bleeding. Patient was instructed to avoid strenuous activity involving the treated limb for the next 24hrs and to call immediately if localized redness, swelling, warmth, pain, or skin changes are noted.

What am I missing on these?

What am I missing on these?
64450 is edit 40 with PT charge 64450-XP-50 for separate provider and remove mod 59 from 97110

Thanks for the info but the therapy is billed under the same provider as the 64450. A PTA is providing the therapy and we bill under the supervising provider. So it still is bundling.
