Wiki BCBS denying excision of Severely atypical moles

BCBS for excisions of severely atypical moles. This is not appropriate, and I have never seen it before. Each has "describe on a pathology exam as needing removal" or similar language. Any ideas on what we can do? Note the benign mole and severely dysplastic mole codes are the same, D22.x where x is site.
