We are getting many claims for LN2 treatments, B9 and pre-malig, denied by BCBS Pathway HMO plans for
"CO-243: Services not authorized by network/primary care providers",
"CO-197: Precertification/authorization/notification absent" or
"Require a preauthorization or referral."

We have an advocacy agent with the AAD looking into this -- is anyone else getting these denials?
