When filing a routine colonoscopy to BCBS Medicare would you use 45378 or a G-code like you would to Medicare?

If BCBS is the Medicare FI or MAC (contractor) then you would use the G code.

If BCBS Medicare is a Medicare replacement policy (in Nebraska it's known as MedicareBlue) you still need to use the Medicare billing guidelines so that they can process the claim in accordance with the Medicare regulations. The exception would be if BCBS has provided you with alternate billing guidelines specific to their Medicare product.
