Does anyone have a template for behavior health treatment plans. We are involved in training audit. We failed first round due to treatment plan and incident to guidelines not being met and made a lot of changes. However we have now failed second round due to treatment plans again. I am trying to find a template to use because I think the issue is we are not specific enough. Even a good website source would be much appreciated. I seem to be going down a rabbit hole this morning of information.
