When Bevacizumab 2.5mg is given Intraocular route, is HCPC code C9257 appropriate for this? Any help would be appreciated!

Are you billing for an OPPS hospital? If so, this HCPCS code would be appropriate. Else, you need to use J9257 as DRGold pointed out.

Note, Avastin is not injected at 10mg for eye disease. Thus, the 10 mg code is not actually correct. Only use that IF that carrier says that's the code they want you to use. Otherwise, it's the J3490 with a narrative description of drug name and dosage. There is no NDC.
