I have a provider who is treating a patient with LT Diffuse Cystic mastopathy of left breast (N60.12) and RT Diffuse Cystic Mastopathy of Right breast (N60.11). This is a chronic condition that has been going on for 2 years. Would they get the points for addressing 2 chronic conditions since it's in both breasts?

Requiring two diagnosis codes to capture laterality does not mean there are two separate conditions.

Each breast does not have an independent treatment plan - the provider’s medical decision-making is based on the overall condition.
