If you need this for work, I'd suggest your facility get a copy of the uniform billing manual. It's the official source for UB04 data specifications. https://www.nubc.org/ub-04-products

If you're just looking to learn as a reference, Noridian has a good table for the type of bill code structure. You can use the table to see how a TOB code is built.

For example, an inpatient hospital claim for admit through discharge would be a TOB 111. This would break down as follows:

1 - hospital
1- inpatient
1 - admit through discharge

But for an outpatient hospital claim, it would be TOB 131.

1 - hospital
3 - outpatient
1 - admit through discharge
