Wiki Billable or not??

Does anyone know if this is billable? I have come across a H & P to code that includes a CC, and an HPI with some PMH, but also states patients refuses,a ROS that states patient refuses, an exam with vitals only and states patient refuses and it has an assessment with plan for the chronic conditions. Thanks

Look into 99201 for new patient and 99211 or no more than a 99212 for established patients.
Of course this having medical necessity and the nature of the presenting problem in satisfied.
