has anyone had trouble billing Aetna a major joint Injxn in the knee, cpt code 20610 (this is for 2014) with Dx code 715.16 and not get paid on the ultrasound code 76942, and if yes how did you resolve it?

Aetna doesn't pay, they consider ultrasound guidance experimental and investigational because it has not been establish that this approach improves outcomes.
