When billing HPV 87621 and it comes back positive and Dr request HPV 16/18 be ran also 87621 x 2 units, how would you bill this ..we billed 87621 x 1 and then billed 87621 x2 with 59 mod they applied these to deduct and and denied the other 87621 x 1 as CO18.
