Wiki billing 93306, 93000 and 99214

Hello everyone
Please help. we have rejects with UNITED healthcare for billing 93306, 93000-51 and 99214. They denied the 93306 for "TC- no details to support" and 93000 denied "multiple procedures reduction". all of these CPT are done on the same date, in the same office by the same physician. we own all equipment. They paid only for 99214. Can someone help with the coding please? I have not seen this with any carrier except United.

Thank you very much heartyoga! I just want to check with you guys before I resubmit as corrected claim. They are the only one who does this..
