In Indiana is it within the scope of practice for a CMA or MA to bill for 99211?

It may be within the scope of an MA to perform services that would be reported by a 99211, but you would bill under the supervising provider, and you have to have met the incident-to guidelines for Medicare as well as the many payers that also follow CMS guidelines.

As with most states Direct Supervision is required and you would bill under the supervisor and not the MA
