Hi there! Hope everyone is doing well.

If I have a mid-level performing a 99232 under the supervision of a physician and the physician signs off on the note, can it be billed under the physician? The patient's insurance does not contract with mid-levels.

No, 'incident to' is not allowed in a facility location, only in the office - this service would have to be billed under the mid-level. The only exception would be to bill as a 'shared visit' which would be allowed as long as the supervising physician also performed and documented a portion of the E&M service, but just signing off on the note would not qualify.

Thank you so much for the quick response. I see where the physician contributed to the note and signed off, but the mid-level dictated most of the note.
