Wiki Billing 99483 in a facility setting

In what sort of "facility setting" can 99483 be billed. Sure, Office (POS 11) is okay, as is ALF (POS 13). SNF (POS 31) apparently is not, tried a few services and all were rejected.

But what about LTC (Long Term Care POS 32)? Can 99483 be billed at an LTC facility, POS 32?

99483 is a physician service code on the MPFS with a site of service differential for facilities, so I don't know of a reason why it couldn't be billed with a facility place of service. What was the reason for the rejection in those other places of service? A rejection could be for other reasons than that the facility setting.

The reason was POS 31 (SNF). Coding edits in our PM software stated not billable on front end, and then Medicare denied on the back end also.
