Hello All!! I work at an allergy clinic where we give allergy injections to patients on a weekly or twice a week basis. We are facing the problem of having a physician present for several days. Normally, we bill a 95115, which needs a physician present. However, I am wondering, can we charge a 99211? The patient comes in and we question them regarding their symptoms and whether they were relieved at all. We give the injection and then they go. We spend about 5 minutes with them. This is performed by either an LPN or RN.

We are a licensed physician office. The physician is not always present and we are looking to expand our injection hours. Per cms guidelines, it is shown that if a physician is present, we would bill 95115 under his NPI as incident to. However, it states that a nurse may give an injection without the physician present, but does not describe the coding aspect. We want to know if we can simply charge E/M 99211 for the nurse evaluation of giving the injection as we do check the patient's allergy symptoms and allergy history each time. If we were able to charge this, we would simply not charge at all for the injection administration.

Did you mean that no physician will be present? And your are licensed to practice as a physician office? Every time you bill a line item with an NPI in the rendering/supervising provider field, that person must be physically present while the patient is there. As far as an injection,no you do not use a 99211 for an injection. You must use the appropriate injection admin code.
