These codes are only for prolonged office or outpatient evaluation and management (E/M) services, with or without direct patient contact, beyond the required time of a primary service, in 15-minute increments. I attached the CMS Fact Sheet regarding these codes.

So far as billing is concerned it will depend on whether the patient's plan allows providers to report monitoring with an E/M code. Medicare requires the G codes. I've seen commercial plans that give providers a choice between the G codes or an E/M office/outpatient code. https://providernews.anthem.com/vir...-reporting-administration-of-spravato-5-10572.

If you report with an E/M code the documentation must support it and the provider will need to document the time for each patient. You'd also have to know whether the payer covers G2212 or 99417 and follow the appropriate time guidelines.
