Can anyone give me any information on the following:

A: Can a Nurse Practioner make a decision for surgery, and is this billable with modifier 57?
B: Does the Surgeon have to examine the patient in addition to the Nurse Practioner on the same day?
C: Where can I find supporting documentation?
